In case of severe injury or emergency, it is best to go to the emergency room and they will call your doctor.

Medication Refills
Please call at least 2-3 days before your medication runs out. This prevents interruptions in your medication schedule. Some medications cannot be refilled by telephone and others cannot be refilled if you are overdue for a follow-up appointment.

Insurance and Fees
Our clinic accepts most major insurance plans and HMOs. Please inquire about your specific plan. We require payment of the insurance CO-PAY on the day of your visit. We will bill you for any deductibles or fees not covered by your insurance plan. Please bring all pertinent insurance information and forms to your first appointment. If your insurance changes, please notify our office immediately to avoid billing problems. Please do not hesitate to discuss with your provider or another staff member.

Medical Records
We will not release information about you without your permission unless required by law (industrial claims, abuse) or in the case of an employment physical paid by an employer. We will provide one copy a year of your records to you, an attorney or another doctor upon written request from you at no charge. Additional copies are available for a $25.00 fee. We will always provide information about immunizations at no charge.

Appointments
We make every effort to avoid delays in the appointment schedule. If you have a long wait it is probably due to an unexpected problem or emergency. Please bear with us. A sincere attempt is made to schedule you with the provider you wish to see. If that provider does not have an opening on the day you requested we will offer you an appointment with another provider in our clinic to accommodate the requested appointment day. Please call us and cancel if you are unable to keep your appointment so we may offer that time to another patient.
